MUSC Health Selects Upstream Healthcare as Its Value-Based Partner for Medicare Patients

Greensboro, NC, November 21, 2022 /PRNewswire/ — The Medical University of South Carolina (MUSC Health) and Upstream Healthcare, a national value-based health care company, have announced their partnership to jointly advance the health and well-being of Medicare patients within their expanded network across the United States. Upstream is a participant in the CMS Innovation Center model called ACO Access, an advanced alternative payment model (APM) for physicians in value-based arrangements, that requires assuming financial risk for Medicare beneficiaries while remaining committed to advancing health equity, stakeholder feedback, and the participant experience. More than 300 primary care physicians caring for more than 30,000 senior citizens are expected to participate in the value-based model.

Upstream and MUSC Health Partnership Aim to Improve Quality Outcomes for Medicare Patients South Carolina By giving them access to a team committed to improving the coordination of care. One goal of the program is to advance health equity for underserved people with a focus on preventive care and management of chronic conditions.
